This is the Privacy and Cookie Statement (“Statement”) of De Fysiovrienden VOF; hereinafter referred to as “De Fysiovrienden”). In this Statement, we explain what personal data we collect and use and for what purpose. We also explain what cookies we use and place. We recommend that you read this statement carefully. This Statement was last updated on May 16, 2018.

This Statement may be amended by us. The most current version of this Statement can always be found on this website. It is recommended that you consult this Statement regularly so that you are aware of these changes.
Personal data
By using our services at www.defysiovrienden.nl, you provide us with certain information. This may include personal data. On the contact form, we ask you to provide the following information:

•Email address
The above information is requested for the purpose of providing you with the requested information and maintaining contact with you. In addition, we record information when you visit our website (including the IP address used) for the purpose of compiling usage statistics and for the security of our website. We also use cookies for this purpose. You can read more about cookies under the heading “Cookies.”
Provision to third parties
Your data will not be disclosed to third parties. With the exception of third parties who are directly involved in the execution of the agreement you have entered into with us or insofar as we are obliged to do so on the basis of a legal obligation.
Security
When processing personal data, Louwers Advocaten maintains a level of security that is appropriate in view of the state of the art and the costs of implementation, in order to prevent unauthorized access to, modification, disclosure, loss, or any other form of unlawful processing of personal data.
Retention period
We do not retain your data longer than is necessary for the purposes for which it is processed, unless the data must be retained longer to comply with legal obligations (such as a retention obligation).
Access, correction, and deletion of your data
If you would like to know what information we have recorded about you, or if you would like to change or delete your information, please contact us at info@defysiovrienden.nl. To prevent misuse, we may ask you to identify yourself adequately, in which case we will ask you to make your photo and social security number on your ID card unreadable. You will receive a written response to the aforementioned requests within four weeks.
Cookies
The De Fysiovrienden website uses cookies. A cookie is a small text file that is sent along with a page from the website and stored on the hard drive of your computer, tablet, or phone via your browser. This allows us to combine different requests from pages on the website and analyze user behavior, among other things.

We use cookies to remember your chosen settings and entered data so that your use of the website is made easier on your next visit.
Website statistics & cookies
This website uses Google Analytics, a service provided by Google Inc. Google Analytics is a web analytics service that uses cookies to analyze how you use the website. The cookies are used to store visitor information, such as the time of the visit to the website, whether the visitor has visited the site before, and which site the visitor came to the web page from. Google may provide this information to third parties if Google is legally required to do so, or insofar as third parties process the information on Google’s behalf.
The information generated about your use of the website is sent to servers in the United States and stored there by Google. We use the option to anonymize the IP address, which means that your IP address is shortened by Google. This makes tracking at the individual level technically impossible.

Privacy
Our practice has private treatment rooms, ensuring your privacy is guaranteed.
In order to provide you with the best possible treatment, Physio Friends keeps a record of your medical and administrative data. This record is subject to the Personal Data Protection Act (Wbp). This act contains rules regarding the purpose of the record, the nature of the data that is recorded, the management of the data, the persons who have access to that data, and the patient’s right of access.
In addition, all our treatments are subject to the Medical Treatment Agreement Act (WGBO). The provisions included here concern, among other things:
the organization of patient files; the patient’s right to access their file; destruction of files; retention period for files;
provision of data to persons directly involved in the treatment or their substitutes;
Since June 1, 2009, the Civil Service Number Use in Healthcare Act (Wbsn-z) has been in force with the aim of combating healthcare card fraud and file mix-ups. This act has introduced mandatory identification in healthcare. Every healthcare provider must verify that a citizen service number belongs to the correct person, and to this end, you must be able to identify yourself with a valid ID. Unlike the general identification requirement, there is no age limit in healthcare. However, children aged 14 and older must show their own ID if they need medical care. An entry for a (minor) child in the parent’s passport can be used to identify the child. This parent must then accompany the child to the physical therapist in order to identify the child. As of June 26, 2012, children can no longer be added to their parent’s passport. However, an entry in a valid passport can still be used as identification in healthcare.
Obviously, Fysiovrienden handles your personal data with care. That is why, in accordance with the above-mentioned laws, we apply the following rules:
•Your data will not be disclosed to third parties (family, occupational health and safety services, insurance companies, etc.). In order to disclose data, the treating physical therapist must have your written approval with your signature. The approval will also be recorded in your file.
•Data relating to children under the age of 16 will be provided to parents or their legal representatives.
•In the event of transfer to other healthcare providers, the report will only be sent/provided if the treating physical therapist has a verbal agreement with you.
•In addition to the physical therapist who is treating you, a limited number of other people also have access to the information contained in your file. These include locums, other physical therapists at the practice, any interns, and administrative staff. All of these people are bound by a duty of confidentiality.
•The data in your file will be kept confidential from unauthorized persons and persons who are not involved in data processing, through the use of authorized access to the file.
•You have the right to view the data recorded about you if you are at least 12 years old. If you believe that data has been recorded incorrectly, you can request the treating physical therapist to change it.
•Health insurers require us to conduct customer satisfaction surveys among our patients. For this purpose, your email address may be passed on to “de Fysio Prestatie Monitor” after your last treatment. If you object to this, please inform your physical therapist.
•Patient data is stored for at least 15 years after the last treatment date.
•If you believe that Fysiovrienden is not handling your data correctly, you can report this via the applicable complaints procedure within the practice.
Third-party websites
Our website includes buttons that allow you to share, “like,” or promote web pages on social networks such as Facebook, LinkedIn, and Instagram. These buttons work using pieces of code that originate from the social media providers themselves. Cookies are placed using this code.
This Statement does not apply to third-party websites that are linked to this website. For the terms of use and privacy and cookie policies of third parties, please refer to the relevant websites.
De Fysiovrienden is not responsible or liable for the manner in which these third parties handle your (personal) data.
Refusing and deleting cookies
You can decide for yourself whether you want to accept or reject cookies, or whether you want your browser to notify you when a cookie is placed. To do this, you need to adjust the settings in your browser.
You always have the option to delete cookies that have already been placed on the hard drive of your computer, tablet, or phone.
In addition to refusing cookies via your browser, you can also install the Google Analytics Opt-out Add-on in your browser. This prevents Google Analytics from collecting information about your website visits.
FRIENDS
De Fysiovrienden is not responsible for friends, all of whom are self-employed professionals, who have their own policies. Contact via them falls outside the scope of our regulations.
